Application for a work visa in Croatia

1/ Personal Profile

• Original passport valid for more than 6 months
• 2 pictures 3.5 x 4.5 white background
• What is the identity card?
• Notarized copy of household registration book of pages with names
• Marital status: married – single – divorced – death certificate

2/ Job profile

• Business registration if you are a business owner or a shareholder – notarized copy
• Tax report of the last 3 months with director’s signature and red seal – original
• Paper of payment to the state budget (quarterly or monthly) – true copy

If you are an employee:

• Labor contract – notarized copy
• Decision to send on a business trip – original
• Appointment decision, if any – notarized copy

3/ Financial records
If the company makes financial payments:

• Company account statement for the last 3 months

If self-pay:

• Duplicate car tie if any
• Please confirm the minimum balance of savings book 100 million VND

4/ If invited to work:

• Invitation letter: clearly state the full name, address, and phone number of the inviter and the invitee in Vietnam (including passport number, date of issue and expiration date). The date of entry, the date of the purpose of the trip, where to go there if financially sponsored.
• Passport face of the inviter
• Contract signed, purchase and sale between two parties, customs invoice….

Note: all scans do not need to send originals.

3/ Financial records

• Notarized copy of company car if any
• Company account statement for the last 2 months

4/ Croatian profile if invited

• Invitation letter: clearly state the full name, address, phone number, pax of the representative of the inviting company and the invitee in Vietnam (including passport number, date of issue and expiration date). Date of entry, date of the purpose of the trip. The invitation has the logo of the inviting company
• Passport photo of the inviter
• In case of financial sponsorship, the following documents must be added: hotel booking, air ticket
• Business schedule
